Indigenous Pharmacist Discharge Navigator Pilot Project

The Pharmacy Department at Thunder Bay Regional Health Sciences Centre (TBRHSC) would like to announce that the Indigenous Pharmacist Discharge Navigator pilot project has reached its end date.

Data was collected, and results are being analyzed for the impact of the project. The results of the pilot project will be disseminated within TBRHSC and externally to advocate for meaningful change in healthcare, including but not limited to:

  • Decreasing barriers and challenges that Indigenous patients experience when accessing healthcare.
  • Improving the quality of care for Indigenous patients and enriching their experience in hospital.
  • Increasing access to pharmacy services in and out of hospital for Indigenous patients from remote and semi-remote First Nations.
  • Increasing the detection and resolution of drug therapy problems that Indigenous patients experience after being discharged from the hospital.
  • Decreasing gaps in communication between Indigenous patients and healthcare providers by ensuring patients are actively involved and fully informed in all aspects of their health care.
  • Active collaboration between Indigenous patients and healthcare providers that allows for shared-decision making that integrates and respects the patient’s values, preferences and goals.
